Under Section 181.105 of the Texas Health and Safety Code, consumers have the right to request access to their health records from “covered entities." To request copies of your health records, you may submit a written request to our office. When submitting your request, please include your full name, date of birth, contact information, a description of the records you are requesting, and how you prefer to receive them, as permitted by applicable law.
If you have concerns about the privacy or security of your health information, Section 181.105 requires covered entities to provide information on how to submit a complaint. You can file a complaint with the Behavioral Health Executive Council (Contact Us – Texas Behavioral Health Executive Council) or the Attorney General (Consumer Protection | Office of the Attorney General).
